0-3 yrs. service - 9 hrs./pay period; 3-6 yrs. service - 10 hrs./pay period; 6-10 yrs. service - 11 hrs./pay period; 10-15 yrs. service - 12 hrs./pay period; 15+ yrs. service - 13 hrs./pay period

Part Time (33-79 hrs/pp)
0.1125 times number of regular hours worked per pay period, up to max. of 80 hours worked.Observed Holidays:

Bereavement Leave Full time employees are eligible for up to 24 hours paid absence due to the death of an immediate family member and 8 hours paid absence due to the death of an extended family member.

Part time employees are eligible for up to 8 hours paid absence due to the death of an immediate or extended family member.

Jury Leave Civista will pay employees the difference between their base wages and jury pay for any missed shifts while fulfilling their civic obligations.

Military Leave All employee will be authorized leave of absence for military assignments.

Family Or Medical Leave Of Absence All employees who have completed one (1) year of service and worked 1250 hours in the previous 12 months are eligible for leave for any qualifying event as defined by the Family Medical Leave Act. Accrued leave will be deducted from your leave bank for all scheduled days missed.

Medical, Dental, Prescription Drug & Vision Coverage All active full-time employees and regular part-time employees (40 to 80 hours per pay period) and their dependents are eligible to participate in the benefit plans beginning the first of the month following the completion of 30 days of service.

Medical Coverage
Civista offers employees two benefit options to help you select the best coverage for your needs.

Dental Coverage
Civista provides Dental Coverage to employees for preventative, basic, major and orthodontia services.

Prescription Drug Coverage
When you enroll in the Civista Medical Plan, you will have access to prescription drug coverage. You may use any participating pharmacy and make the required co-pay.

Vision Care Plan

You are entitled to one free eye exam per family member in a two year period provided you visit a participating eye care center.

Life Insurance/
Accidental Death & Dismemberment
Basic Life Insurance (Employer Paid)
Life Insurance provides financial protection for your family in the event of your death. Civista provides you with Basic Life Insurance equivalent to 1 times you basic annual salary rounded to the next higher $1000 at no cost to employees scheduled 40 hours of more per pay period.

Additional Life Insurance

Civista offers two separate options for purchasing additional life insurance for yourself or your family.
Employees who are eligible for the Basic Employer Paid Life insurance can purchase additional term life insurance on yourself, your spouse and your children.
Permanent life insurance can be purchased through Benefits By Choice, Inc. This coverage is also available to your spouse, children and grandchildren.

Short and Long Term Disability Short Term Disability
Employees are eligible to purchase short term disability though Benefits By Choice, Inc. This benefit protects up to 66 2/3% of your income in the event you can not work due to a covered accident or illness (maternity is included). Benefits begin after 14 calendar days and pay until you can return to work or for 3 months. Money received from this plan is tax free and typically used for necessities such as rent, mortgage, groceries, etc.

Long Term Disability

Civista offers long term disability to employees who have 90 days of continuous disability and have been employed in a benefit eligible category (64 or more hours per pay period) for more than 6 months. Employees are entitled to 60% of their monthly income (up to $6,000.00 monthly maximum).

Personal Cancer Protection
Survival rates and expectations for cancer grow each year but so do the costs of new and advanced treatments. Civista offered employees through Benefits By Choice, Inc., a cancer protection program that pays tax-free benefits directly to you regardless of your health insurance. Lost time, travel, out of network expenses and other out-of-pocket costs are offset by this type of benefit. Coverage is portable and you may protect yourself, your spouse or the entire family.

Employee Assistance Program (EAP)
Pension Plan

Civista offers 24-hour free professional, confidential support services to employees and their dependents through Ceridian Lifeworks. These services are designed to help people cope with a variety of personal and career-related issues at no out-of-pocket expense.

As a Civista Medical Center employee, you are required to contribute nothing to the Pension Plan. You will receive a pension allocation each calendar year in which you are paid for at least 1,000 hours. Civista Medical Center will credit your retirement account with a percentage of your total W-2 pay based on your years of credited service (a calendar year in which you are paid for at least 1,000 hours). Civista Medical Center’s base allocation to your retirement account will increase with your years of credited service. An employee is fully vested after five years of service in which you worked 1,000 hours. (Please refer to the Summary Plan Description for more details.)

Tax Sheltered Annuity Plan (TSA)
As an employee of Civista Health, you are eligible to participate in a special benefit known as a Tax-Sheltered Annuity (TSA) starting the first day of your employment. A portion of your salary may be set aside voluntarily in a tax-free interest bearing annuity program. After one year of employment, there is a matching contribution by Civista per schedule not to exceed 2% of income for regular employees. The TSA is a long-term retirement investment program that offers you the following benefits:

• Flexibility to choose your contribution amount (subject to tax code limits)
• Reduction in current federal taxes because you do not pay federal tax on the money until you withdraw it during your retirement years when most retirees are in a lower tax bracket.
• Deferral of current income taxes on interest and investment earnings.
• Automatic payroll reductions.
• Withdrawal of funds after age 59 ½ without the additional 10% tax penalty.

Flexible Spending Accounts Health Care FSA
This plan offers a unique way to save money on taxes for health care expenses for you or your family by allowing you to pay for eligible Health Care expenses with pre-tax dollars.

Dependent Care FSA

You may use pre-tax dollars from your Dependent Care FSA to pay expenses up to $5,000.00 for services that enable you and your spouse to work outside the home.

Tuition Assistance
After completion of six (6) months of employment, active full-time employees and regular part-time employees (64 to 79 hours per pay period) may be eligible for 75% reimbursement for tuition and lab fees following successful completion (Grade C or better) of a career-related course up to a maximum of $1,850 per calendar year ($600 for part-time employees). All requests for tuition reimbursement must be submitted to your department manager after completion of the course.

Continuing Education After completion of six (6) months of employment, active full-time employees and regular part-time employees (64 to 79 hours per pay period) may be eligible for registration reimbursement up to $500 per calendar year and up to 24 hours paid absence for continuing education. Part time employees (24-63 hours per pay period) are eligible for registration reimbursement up to $100 per calendar year and up to 8 hours paid absence for continuing education. Requests for continuing education must be submitted in advance to your department manager.

Credit Union Civista is pleased to be a member of Cedar Point Federal Credit Union. Unlike many other financial institutions, credit unions are not-for-profit and there are no outside stockholders. Credit Union members are actually the owners of the credit union. This means all excess profits are returned to the membership in the form of higher saving rates, lower loan rates and lower fees.

Cedar Point Federal Credit Union is on-site in the hospital every payday from 10:00 am to 12:00 pm (excluding Federal Holidays). Cedar Point offers the following services:
