Maintenance of Health Information:
The Health Information Management Department maintains hospital medical records of the following types of permanent retention:

  • Inpatient Hospitalizations
  • Same Day Surgery
  • Emergency Department

Release of Health Information:
All patient health care information at Civista Medical Center is confidential. Therefore, medical records cannot be released to any person without the authorization of the patient or the patient’s legally authorized representative (unless authorized by law).

Requesting Medical Records:
All requests for the release of medical records must be submitted in writing and must be dated and signed by the patient or the patient’s legally authorized representative. In the case of a minor, the parents or guardian must sign the authorization.

Submitting a Request:
Complete the Medical Records Release and Authorization Form and

    Attn: Medical Records Correspondence or
  3. Hand Deliver:
    Health Information Department located in Civista Medical Center. Open to the general public: Monday – Friday, 8:30 – 4:00 pm
    Open to the medical and hospital staff

    Monday – Friday, 6:00am – 6:30pm
    Saturday and Sunday 8:00am – 4:30pm

Depending on the nature and size of the medical records request, your medical records may require processing through MRO, a contracted company with Civista Medical Center. If processed through MRO, you will be contacted by them for payment. Records are mailed upon receipt of payment – patients should allow 10 business days to process the request. All records are mailed via U.S. Mail to the address specified on your request.

Birth Certifications:
The Health Information Management Department is responsible for the creation of a birth certificate for all live-born infants delivered at or in route to Civista Medical Center. Birth Certificates are submitted within five days of birth to the Department of Maryland Vital Records, who is solely responsible for distributing copies to qualified individuals.
